TRANSPORTATION WAIVER
Leigh Community Schools has created a waiver form for parents requesting that their student athlete in grades 9-12 be authorized to drive to a practice or home game in Clarkson or Dodge (softball). The date of the requested private transportation and signatures from parent and student are required.
The transportation waiver is to be submitted to the administration at least one day in advance of the practice or game. This form replaces hand written notes, emails, phone calls, and texts.
